New SDL_MSBIndex, updated VS projects, cleanup gamecontroller.

- Updated Visual Studio 2010 project with SDL 2.0 library names
- Don't use a 256 byte LUT, but rather <32 bytes of data on the stack.
- Cleanups in SDL_gamecontroller.h
